We ask that you complete your forms prior to your appointment to save valuable time. You can bring the printed completed forms, send them via e-mail, or upload them below.
We can usually schedule you to be seen within 24 hours of your first phone call. Below are some things you can expect when getting started:
FIRST VISIT
A bit of paperwork
We ask that you fill out forms that provide us with general information about you and your condition. We are happy to assist you with the paperwork. We also ask that you save about 20 minutes by downloading them and filling them out before you come in.
Consultation and History
Dr. Levitan will sit with you to review your initial paperwork and discuss your current health concerns as well as find out more about you and your healthcare goals. By understanding what you like to do and what is important to you, Dr. Levitan can focus on getting you back to doing those things that are important to you.
Examination
In order to assess your body’s structure and function the doctor will perform a variety of targeted non-invasive tests that will determine the causes of stress and injury to your spine and nervous system. If deemed necessary you will be referred to a local radiology facility for x-rays.
After the consultation and examination Dr. Levitan will determine if Chiropractic care can be of help to you. If so, he will formulate and start an appropriate treatment plan in order to obtain the best results.
SECOND VISIT
Report of Findings and Treatment
During the second visit Dr. Levitan will review with you the findings of the first visit. He will offer his recommendations and you will both then discuss your spinal and wellness care options. Once treatment goals are agreed upon, Dr. Levitan will perform various gentle Chiropractic Adjustments to start correcting Vertebral Subluxations and start reducing your spinal and neurological stress.
Home Instructions
Before leaving, the doctor may give you instructions on certain activities or procedures to be conducted at home. This may include ice or heat applications, avoidance of certain activities or positions, as well as home stretching and exercises.
